Stay Active! Find a friend to
walk with you for 30 minutes at least three times a week.

Get a Hemoglobin AIc test from your
health care provider every 3 months if you use insulin/every 6
months if you do not use insulin.

Have a dilated eye exam every year
from an eye doctor.

Have your cholesterol/HDL/LDL and
triglycerides checked at least once a year.

See your dentist twice a year.
Brush and floss every day.

Have a Micro albumin test every year
to check kidney function.

Get your flu shot every year.
Keep all other immunizations up to date

health professional
